, or call 1-877-773-3355 to request that one be faxed or mailed to you. Follow the instructions listed on the form. Mail the completed form, along with a copy of your packing slip, to the address listed on the form by the required postmark date provided on the form.
Q: Do I need to complete more than one form if I qualify for multiple rebates?
A: Separate forms are required for each Software and Peripheral rebate offer.
Q: To what address should I send my submission?
A: Mail your completed Rebate Redemption Form, along with a copy of your packing slip, to the address provided on the form.

A: Your packing slip was delivered with your system in the clear, plastic envelope affixed to the outside of your package.

Q: What's the deadline for sending in the Rebate Redemption From?
A: Your completed Rebate Redemption Form along with a copy of your packing slip must be postmarked by the required postmark date listed on your qualifying system rebate form. Any claims postmarked after the required postmark date will not be processed.
